Each of these are available for individual download as well.

In addition, you will be receiving two phone calls from the center prior to your surgery.

One will be from our Nurse Screener, who will go over your medical history and provide you with important information.

The other call will be from our Business Office regarding estimated insurance coverage and out of pocket expenses.
